I created a client account and added a domain. But I'm unable to create MS SQL database. The drop down box shows only MySQL.

You cannot create MS SQL databases with Plesk. As far as I know it is only given to us as a convenience for those of us that need it. There are numerious other posts about how to use some form of Enterprise manager to connect to your MS SQL databases. Please read them.

How can I (my customers) create a MS SQL database and the associated user if the option for MS SQL database creation is not available?

You can access MS SQL database using enterprise manager but the database should be created first. If that option is available form the control panel then it is useless. I'm currently using a server with PLESK and I can create a db from the control panel.

If you have the windows power pack or you have the mssql addon then it would appear you can..

But i don't know that that is included. I didnt notice that option when I was participating int he beta program.

It is included. You just need to add your settings to the sql server database screen and then I believe you can create databases and users
